HFSA Outstanding Heart Failure Care Team Award

The HFSA Outstanding Heart Failure Care Team Award recognizes one or more interdisciplinary teams that exemplify excellence in the delivery of heart failure care. This award honors teams whose collaborative approach, innovation, and patient-centered focus have improved outcomes, advanced best practices, and set a standard of excellence in the field. 

• Open to all heart failure care teams in the U.S. that has made a collective accomplishment in the field of heart failure

Evaluation of nominees and selection of the recipient will be chosen by a majority vote of the Awards Committee and submitted to the HFSA Board of Directors for approval.

• Each nomination should consist of a letter of recommendation from a nominator highlighting the collective accomplishments of a heart failure care team.
• Nominations may only be made by HFSA members in good standing. Self-nominations are accepted.

2024 – Center for Advanced Heart Failure and Mechanical Circulatory Support Team at Rochester Regional Health and Heart Failure & Transplant Team at Intermountain Medical Center

2023 – Texas Children’s Hospital Pediatric Heart Failure Team

2022 – VCU Pauley Heart Center Nurse-Led HF Disease Management Team
