Remote Care in Heart Failure Post Pandemic

The American College of Cardiology/American Heart Association/Heart Failure Society of American 2022 guidelines for the care of patients with heart failure (HF) recommend a multidisciplinary team approach for patients with HF because the multidisciplinary HF approach reduces the hospitalization rate for HF, reduces healthcare costs, and improves adherence to self-care. This session will discuss the optimal multidisciplinary team structure and each team member's delineated role, institutional goals and metrics for HF care. It will also discuss how to apply the multidisciplinary care approach in specific settings. Speakers will discuss the challenges and strategies to improve health disparities in rural areas and share practical skills that HF clinics in the nation can incorporate in their practice.
