Inside the 2025 ACC Cardiogenic Shock Guidance: 5 Key Highlights from Dr. Shashank Sinha

In the latest episode of Heart Failure Beat, Dr. Shashank Sinha offers a deep dive into the newly released 2025 ACC Expert Consensus Statement on Cardiogenic Shock—what he calls a “team of teams” approach to one of cardiology’s most dynamic conditions.

Here are five key highlights:

  1. Early Recognition is Essential
"Call early and call often." Dr. Sinha emphasized that early suspicion and staging are critical to prevent rapid decompensation.

  2. Invasive Hemodynamics Still Matter
Despite historical debate, new observational evidence and ongoing trials like PAX are reviving confidence in the pulmonary artery catheter. "It’s one of the simplest and most valuable things we can do," he notes.

  3. Use the S.U.S.P.E.C.T. Mnemonic
From Symptoms to Triage, this new tool gives clinicians a practical way to identify shock quickly and consistently.

  4. Shock Teams Save Lives
"It takes a team of teams." A coordinated, multidisciplinary approach ensures patients receive the right therapy at the right time.

  5. Reassess, Reassess, Reassess  "You can’t just round in the morning and hope for the best." Frequent reassessment is essential as up to 90% of patients worsen during hospitalization.
