Eligibility
- HFSA members will nominate patients or caregiver for the award
- Nominee must be an HFSA patient/caregiver member (if not currently a member of HFSA, membership enrollment is complimentary for patients and caregivers)
- Past winners of the award are not eligible
Evaluation of nominees and selection of the recipient will be chosen by a majority vote of the Awards Committee and submitted to the HFSA Board of Directors for approval.
Nominations
Each nomination should consist of:
- Previous nominees can be re-nominated (new submission is required) and posthumous individuals may be nominated
- Cover letter including contact information of nominee (name, address, phone, and e-mail)
- A summary of accomplishments addressing each of the evaluation criteria (maximum length one page)
Nominations may only be made by HFSA members in good standing. Self-nominations will not be accepted.
